Consent Agreement for Electronic Disclosures and Communications
You are viewing this page from . is a consumer website operated by Oinkbound, and this page applies to it.
Last Updated: effective date
Attorney review this whole page. Open question: does any law require Oinkbound to give consumers a notice "in writing," so that the federal E-SIGN Act consumer-consent steps apply? If not, keep this page as a plain statement of how records are delivered.
Please read this agreement carefully, and print or save a copy for your records. Words that start with a capital letter and are not defined here have the meaning given in our Terms of Use.
Introduction
When you submit a Request on one of our Sites, you are asking Oinkbound to connect you with one licensed insurance agent about health coverage. Our service is free to you and takes place online and by phone. To provide it, we need your agreement to give you notices, disclosures, agreements and other records electronically ("Disclosures and Communications"), and to do business with you electronically. This agreement explains how that works. Our Terms of Use are part of this agreement.
Electronic Signature
Clicking the submit button on a Request Form, with the consent wording shown next to it, is your electronic signature on that consent and on the agreements it refers to. It has the same effect as signing on paper. Attorney review
Electronic Disclosures and Communications
We will give you Disclosures and Communications electronically, by showing them on our Sites, by sending them to the email address you give us, or both. This includes this agreement, our Terms of Use and Privacy Policy, the consent you agree to when you submit a Request, and any notice the law requires us to give you about your Request.
You may ask for a paper copy of any Disclosure or Communication at no charge. Email support email address or write to legal entity name, Attn: Disclosure Request, mailing address, and tell us which document you want.
Scope of Consent
Your consent covers the Request you are submitting now and any later Request you submit to Oinkbound, and Disclosures and Communications from Oinkbound and from the licensed agent who receives your Request about that Request.
Consenting to Do Business Electronically
Before you agree to do business with us electronically, please check that you have the hardware and software described below, and that you can open and keep the records we send you.
Hardware and Software Requirements
To receive, view and keep Disclosures and Communications electronically, you need:
- a computer, tablet or smartphone with an internet connection;
- a current version of a common web browser, such as Chrome, Edge, Firefox or Safari, that supports encrypted (HTTPS) connections;
- an email account you can access;
- software that can open PDF files, which is built into most current browsers and devices; and
- a printer, or enough storage space on your device to save copies.
If these requirements change in a way that could stop you from opening or keeping our records, we will tell you and give you the chance to withdraw your consent without charge.
Withdrawing Your Consent
You may withdraw your consent to receive Disclosures and Communications electronically at any time, free of charge, by emailing support email address or writing to us at the address above. Tell us your name, the phone number and email address you used, and a mailing address for paper records. Withdrawing this consent takes effect after we have had a reasonable time to act on it, does not affect records we gave you electronically before then, and does not by itself withdraw your consent to be called or texted. To stop calls and texts, see Do Not Call.
Changes to Your Contact Information
Please tell us if your email address or mailing address changes, so you keep receiving our Disclosures and Communications. Email support email address or call phone number.
PLEASE PRINT OR SAVE A COPY OF THIS AGREEMENT FOR YOUR RECORDS.
